Fee Schedule:

Just like a doctor’s office, we often do not know the best course of treatment for you until after we see you.  We provide a copy of our Fee Schedule to all our New Patients.  It is also available upon request.  Please call or email us if you’d like to see our current Fee Schedule.

General Financial Policies:

We are happy to process paperwork for Motor Vehicle Accident (MVA) claims.  Most persons with MVA injuries do not have to pay anything out-of-pocket to see us.

We do not process health insurance claims.  Upon request, we are glad to provide a superbill, which you can then submit to your health insurance company for reimbursement.

We require a deposit (which may be equal to the total cost of the appointment) for appointment confirmations.  This helps us all to be committed to what we’re going to do together, and begins the process of healing.  (We currently waive the deposit requirement for Motor Vehicle Accident injury patients, because their auto coverage pays for their care.)

Initial Appointment Deposits:   

  1. New Adult Patients:  $150 
  2. New Pediatric Patients:  $97

Follow-up Appointment Deposits:

  1. Established Adult Patients:  $97
  2. Established Pediatric Patients:  $47

Deposits may be paid over the phone or internet by credit card, debit card, or Paypal.   We provide full transparency of all our service fees.  If the cost of the appointment exceeds the deposit, the remainder is due immediately upon treatment conclusion.  Payment at the time of service can be made by cash, check, credit/debit, or Paypal.  A $40 NSF charge applies to returned checks.  A 100% refund is provided for cancellations with at least 48-hours notice. The fee for missed or late-cancelled appointments is $150 or the typical cost of the appointment, whichever is less.


We provide the following discounts on services:

  • Families with Young Children (age 6 and below):  10% off
  • Local Neighborhood (Old Town Historic, Longmont):  10% off
  • Fellow Post-911 Veteran:  10% off
  • Chiyu Center Certified Usui Reiki Practitioner:  10% off

Motor Vehicle Accidents (MVA):

We do an EXCELLENT job getting you back to A+ health after a Motor Vehicle Accident!  If you’ve been injured in a MVA, you will need to fill out a special form which will be uploaded to your Electronic Health Record.  (If necessary, you can opt to fill it out when you arrive for your initial appointment.)  Please be prepared to provide detailed accident information, including policy and contact information for your automobile insurance company.  We will also ask for the other driver’s insurance and copies of insurance cards.  At a minimum, we need the driver’s full name for the car in which you were riding, the insurance company’s name and phone number, and the policy or claim number. 

In nearly all cases, you will not be required to pay anything for treatment.   We will submit bills to your automobile insurance company for covered services on your behalf.  We will call to verify insurance, coverage limits, and any requirements your company needs from us.

Your MVA treatment can only cover conditions that were caused or worsened by the accident.  Your automobile insurance policy will not cover services such as nutritional consultation or Integrative Health Analysis.  It will also not cover herbal medicine or homeopathic products.  Please be aware that some companies will deny claims for multiple care providers on the same day.  In other words, they will not reimburse for acupuncture and a chiropractic adjustment on the same day.  We recommend you always schedule your appointments on different days in order to avoid denial of payment.  If your insurance policy or legal action does not pay for your treatment for any reason, you will be responsible for all accrued charges for services.  MVA policies do not cover missed appointment fees; the responsibility for those fees belongs to the patient.  Please keep this in mind when making care choices.

Health Insurance (Blue Cross/Blue Shield, Aetna, etc):

If your health insurance covers acupuncture and you want to use it, that’s great!  You will make an appointment and pay a deposit just like a regular cash/credit patient.

After we see you, we’ll give you a super-bill that you can submit to your health insurance company for reimbursement.  If you desire a super-bill, you MUST inform us prior to your initial appointment.  

Please know that we are not in-network with any health insurance companies.  We do not call to verify coverage nor do we process health insurance claims.  We’ve made this choice consciously, as health insurance falls short of authorizing necessary treatment for people who need it, nor does it reimburse us appropriately for our care.   We refuse to rush people through our office like cattle.  And we can’t spend hours jumping through their hoops.

We like to work directly with our people in a clean, simple transaction, where you pay us for helping you just like you’d pay any other professional.  At the same time, we understand that if you have benefits, you probably want to use them.  So we do our best to meet you half-way by providing a super-bill.

We also understand that you want to know your chances of reimbursement.  Here’s our experience:  Most of our patients do receive at least partial reimbursement.  However, there is no guarantee that your insurance company will reimburse you, either fully or partially.  We can accept no responsibility for a denied claim for any reasonConfirming coverage, staying within your limits, and working with them is entirely your responsibility.  We will be happy to support you as much as we can.  If you need us to call them, draft letters to them, etc, we will bill you appropriately for our time.

In general, we recommend that patients don’t “count” on reimbursement.  We suggest you consider it a “happy bonus” if your health insurance reimburses you!  We hope they will!

Health Savings Accounts (HSAs):

We positively encourage all our folks to use Health Savings Accounts (HSAs) at our clinic.  Typically, all you need is a receipt from us in order to use your HSA funds.

Every company is different.  Please call your HSA representative to confirm what is, or is not, covered by your policy before your initial appointment.  Be sure to let us know exactly what your company said, so we can do our best supply you with what you need.  To date, all patients who desired to use their HSA with us were able to do so with no problem!   

Refunds/Exchanges/Cancellations/Late Arrival:

A 100% refund is given for cancellations with at least 48-hours notice.  Patients are responsible for a $150 fee or the typical cost of the missed or cancelled appointment with less than 48-hours notice.

Our services and products are of the highest quality and value.  In any medical treatment, there is no guarantee of improvement or cure.   There is no refund on services rendered, custom-ordered products, or any opened products.  Products in re-saleable condition may be accepted for return at provider discretion.  Services purchased as part of a treatment plan may be exchanged for other services (for the same patient) at provider discretion, and cost will be adjusted accordingly.  If you purchased a treatment package or  plan, you may cancel it at any time, and any remaining appointments (with at least 48-hrs notice) will be fully refunded to you.  Services purchased as a package are not transferable to other persons.

If you arrive more than 15-minutes late to your appointment, it may be cancelled at provider discretion.  In such cases, the missed/late-cancelled appointment policy applies.  Please call or text if you suspect you will be more than 10 minutes late.  We’ll do our best to accommodate, but if the schedule does not allow it, we may have to cancel according to policy.

Supplements, Herbal Medicine, & Fascia Testing:

We do not keep a large inventory at the clinic.  Most products are ordered on an as-needed basis.  Shipping and handling charges are passed along to the patient.  Products may be shipped directly to your desired address.  Sometimes, your provider suggest joining a group order for multiple patients at once, which you can pick-up at the clinic.  Sometimes, with a large enough order, shipping will be free.  Free shipping is a perk or bonus and is never guaranteed.  Patients are responsible for the percentage of shipping or handling charges that belongs to them.

Erika Marie offers a unique form of fascia testing for products.  It is not muscle testing, but a more subtle art that examines your body’s response on the energetic-physical level.  Fascia is the tracing-paper-thin connective tissue throughout the body, that covers organs and connects all body systems.  The current best “physical” explanation for acupuncture is that the acupuncture points connect to the fascia, as impulses travel through the fascia to organs and other fascia points faster than impulses travel through nerves.  Whenever possible, we will test supplements and herbal medicine to see if your body responds favorably to it.

It is not possible to fascia test custom-ordered Chinese herbal medicine formulas at this time.  We order custom-Chinese herbal formulas through Urban Herbs, a Chinese herbal medicinary located in Avon, Colorado.  It is simply not possible for us to maintain a stock of every potential herb (there are over 300 Chinese herbs) at this time.

When a product or herbal formula is not available to test, we make recommendations based on our best clinical judgment, education, and experience.   Typically, we find we were “spot-on” with our selections.  However, we can make no guarantee that your body will “test positive” for something we do not have on-hand.  We do not accept returns on custom Chinese herbal formulas or any opened items, irrespective of fascia testing results.

We appreciate your understanding that we are doing our very best by you.  We use fascia testing to confirm our education and experience-based recommendations, not direct them.  Very few providers in the nation offer this unique sort of testing.  While we’ve come to trust it, fascia testing is not the sole determinant of what is right for you.  We recommend you keep any items for which you no longer test “positive” until their expiration date.  Our bodies rebuild cells constantly and change radically every few months.   What might not be right for you now, may be perfect for you in the future.
